Taylor Swift minor stage collapse in Brisbane in December 2013 as reported in themusic.com.au:

A minor staging collapse affected audience members in the most expensive sections of Saturday night's Taylor Swift show at Brisbane's Suncorp Stadium, forcing those closest to the stage to be evacuated from their prime position.

This story in CX magazine outlines a stage roof being used in Taree, NSW without the requisite engineers certification.

Unsafe toys have been found in the UK.  With claims that up to 40% of electronic toys fail toxin safety tests.

September 2015:  CX Magazine reports that certification has been questioned for a stage roof from a NSW hire company.

Zip ties are the new gaffa for dodgy technicians

The joke used to be for dodgy technicians: if in doubt use gaffa tape. Now it's zip ties.  This projector is flown from the ceiling, upside down, and hung with zip ties.

Cup hooks holding up a speaker

The commentary that arrived with this photo is that the ceiling clearly had four patched holes where a similar set up had fallen out, but still someone has fixed the speaker with cup hooks.  The speaker is also not at the right angle and is immediately above customers' heads.  This was in a cafe.

If in doubt safety pins ...
Someone has decided that the curtains need to be pinned back at the wall.  And so they've pinned them to the power cable. The consequences would be interesting.

DIY your own bolt
This homemade bolt was holding speakers in place at a high school. Yes, the speakers were flown. Now removed, we are pleased to say, this bolt has been replaced with something that is safe and correct.
